Tree Interface for County_State_City

Hi,

I was following this link to set up the tree interface.

I would like to have Region --> State --> City tree interface.

However, the output was not I was expected.  There are multiple cities missing under state.

I thought the problem is due to the tile tool. Using the tile tool creates only 10 categories because there are only 1-9 numbers.  However, when I looked at the West region using the interface designer, there are 12 states on the list.

Does anyone know how to deal with this problem?
